Position Summary
The Wellness RN plays a key role in promoting client health, safety, and well‑being through in‑home wellness visits. This role is ideal for a nurse who values autonomy, meaningful one‑on‑one care, and a predictable 4‑day‑per‑week schedule.
Responsibilities
- Conduct in‑home assessments and routine health evaluations
- Monitor vital signs, medication compliance, and overall client status
- Provide education to clients and families on health maintenance and safety
- Collaborate with the care team to support individualized care plans
- Identify changes in condition and communicate promptly with supervisors
- Promote independence, dignity, and quality of life for each client
Qualifications
- Active RN license in the state of Maryland
- Minimum 1 year of nursing experience (home health or community care preferred)
- Strong assessment and communication skills
- Ability to work independently and manage a flexible caseload
